Croatia's Football Federation (HNS) faces fresh disciplinary proceedings over incidents during Monday night's 1-0 defeat by Spain in Gdansk.
The HNS was fined £65,000 by Uefa on Monday after fans racially abused Italy striker Mario Balotelli.
Uefa has opened a new improper conduct case over racist banners displayed by fans during the Spain game.
The Federation also faces punishment over the conduct of its team after six players were booked on Monday.
A Uefa statement read: "Uefa has today opened disciplinary proceedings against the Croatian Football Federation (HNS) for the setting-off and throwing of fireworks, and the improper conduct of supporters (display of racist banners and symbols) at the EURO 2012 Group C match against Spain in Gdansk on Monday.
"The disciplinary case also involves the improper conduct of the team (six Croatian players received a yellow card during the match)."

Uefa will deal with the new case on Sunday.
It is the third time at Euro 2012 that Croatia's Football Federation has been in trouble over fan behaviour.
Last week, the HNS was fined £20,000 after supporters let off flares during Croatia's 3-1 win over Republic of Ireland.
Then on Monday, Uefa imposed a further fine over the abuse of Balotelli during Croatia's 1-1 draw against Italy.
A photographer reportedly saw a steward retrieving a banana and heard monkey chants directed at the Italy and Manchester City striker.
Fare (Football Against Racism in Europe) announced that its observers heard between 300 and 500 Croatia fans chanting monkey noises, which prompted Uefa to investigate.
That punishment also related to the setting off and throwing of fireworks.
But the scale of the fine was £15,000 less than the £80,000 Uefa gave to Denmark striker Nicklas Bendtner for revealing sponsored pants.
That sparked criticism from Manchester United defender Rio Ferdinand and Manchester City captain Vincent Kompany.
The HNS, which last week labelled its misbehaving fans "deviant", has been previously fined twice for racism by Uefa and Fifa since 2008.
Croatia's defeat by Spain saw them knocked out of Euro 2012, finishing third in Group C behind the reigning champions and Italy.

Racism is a Learned Cancerous Behavior that requires support by others
Human Beings are flawed entities, yet, Humans can control their inequities simply by "choosing to".
A Racist is any individual or group of individuals that uses mal behavior to intimidate, mistreat, abuse, bully, humiliate, degrade, offend, and in some cases, kill people who they deem as not worthy of equal and fair treatment, people they deem as unequal primarily because of the color of their skin.
Those that are Racist, are taught to be racist by their immediate family, friends and community. It takes a family, a group of friends, a community and a society to provide the foundational environment for the racist to exist.
The Racist mind, when put in isolation, dissolves, becomes confused as to why the racist is alone. The Racist cancerous behavior require support from others, just like a disease that needs flesh to eat.
We feed the racist their strength and power by either joining in with their ignorance and wickedness or by doing nothing as they abuse, bully, intimidate, assault or kill the object/s of their learned hatred.
It is us that have the right and authority to remove the power of a racist by denying the racist its power by verbally condemning the racist immediately, or calling the authorities and informing them of the acts and actions of the racist.
Children who bully others are punished by placing the bully in a corner, banning the bully from playing with others, and spanking the bully for their unacceptable behavior. All of these actions are specific to corrective actions used to point out to the bully that his/her actions are wrong and if they don't change, more punishment will be forthcoming.
Anywhere in the world, a racist must be challenged by all those that surround the racist, and if the racist is surrounded by racists, then all the racists that support the abusive racist should be punished as well.
Mal behavior can only be corrected by immediate action by the fair minded ones surrounding the racist.
It doesn't matter where a racist exists, he/she is surrounded by supporters who are either racist or are supported by people who condone racism.
Everyone has a mirror, and the mirror can't be lied to.
Croatia racism is only another story of humans who are weak, humans that need to be accepted by others so they learn the behavior of racism, as a human need to be accepted.
The Weak Mind needs acceptance and it accepts anyone or anything that will give it a sense of acceptance, to include, being accepted as a racist.
